Cancellation and Reschedule Policy

BEFORE YOU SCHEDULE AN APPOINTMENT, PLEASE
REVIEW OUR CANCELLATION & NO-SHOW POLICIES

At our Crystalline Clinic, we understand that sometimes unforeseen circumstances arise that require you to cancel or reschedule your appointment. However, please be aware that our time is valuable and we have reserved a time slot specifically for you. Therefore, we kindly ask that you provide us with notice of any cancellations or rescheduling requests as soon as possible.

Cancellation Policy:
  • If you need to cancel or reschedule your appointment, please provide us with at least 48hr hours notice. This allows us to fill your appointment slot with another patient who may be waiting for an opening. You are only able to reschedule ONCE; if you are unable to attend your rescheduled appointment your booking will be cancelled.
  • If you fail to provide us with at least 72 hours notice, or do not show up for your appointment, you will be charged a cancellation fee equivalent to 50% of the scheduled service.
  • If you arrive late for your appointment, we will do our best to accommodate you, but please understand that your appointment may need to be shortened in order to stay on schedule for our other patients.
  • If you are more than 15 minutes late for your appointment, we reserve the right to cancel your appointment and charge you the full amount of the scheduled service.
  • If you have purchased a package of services, the cancellation fee will be applied to one of the services in your package.
  • DEPOSITS ARE NON REFUNDABLE UNDER ANY CIRCUMSTANCES!!!!

We understand that emergencies can happen and we will do our best to work with you in the event of an unexpected situation. However, we ask that you please respect our time and other patients by providing us with adequate notice of any cancellations or rescheduling requests.
